Registration and paperwork for your visit are handled through HealthPartners, so there is nothing to download here.
Before your appointment
If you have a HealthPartners or Park Nicollet online account, you can complete your registration, update your medical history, and review your medications ahead of time. Doing this in advance usually means less time in the waiting room. If you do not have an account yet, you can create one at healthpartners.com.
If you would rather not use the portal
Arrive 10 to 15 minutes early and we will have you complete a short health questionnaire at the front desk.
What to bring
- Your insurance card
- A driver's license or valid photo ID
- A referral letter, if your plan requires one
- Any relevant imaging or records (X-rays, MRI, CT)
- A list of your current medications
